Dexcom CGM Sample Program

Terms & Conditions

1. The Promoter

The promoter of this program is New Zealand Medical and Scientific, doing business in New Zealand with offices at "2A Fisher Crescent, Mount Wellington, Auckland 1060" (“NZMS”). 

2. Scope of Program and Eligibility

The title of the program is “Dexcom CGM Sample Program” (the “Program”). Eligible individuals can receive one (1) free Dexcom ONE+ OR one (1) free Dexcom G7 Sensor in accordance with these Terms and Conditions (“T&C”). 

Eligibility requirements: 

• This Program is open to New Zealand residents aged 18 and over with type 1 or type 2 diabetes aged 18 and over. Parents/guardians of children with type 1 or type 2 diabetes aged between 2 and 18 are also eligible.
• Individuals who have not previously used or tested the Dexcom ONE+ OR Dexcom G7 system in the past 12 months.
• Individuals who have a mobile device compatible with the Dexcom ONE+ OR Dexcom G7 app. Please check the device compatibility guide, which can be found here .

By submitting a claim to this Program, you confirm that you are eligible to claim one (1) free Dexcom ONE+ OR one (1) Dexcom G7 Sensor. NZMS may require proof of eligibility and identity to prevent fraudulent activity. 

NZMS will not accept claims that do not comply with the eligibility criteria or where the individual has previously entered or received a Dexcom ONE+ OR  Dexcom G7 Sensor. NZMS reserves the right to disqualify claims that are contrary to the spirit or intention of the Program. 

Prior to claiming, ensure you have read and understood the purposes for which the Dexcom ONE+ OR Dexcom G7 System should be used. This information is available on the Dexcom website . 

There is a limit of one (1) claim per person. Entries on behalf of another person will not be accepted, and joint submissions are not allowed. Claims on behalf of a dependent (child, relative, or other) are allowed as an exception. Please provide the personal details of the individual who will be using the Dexcom CGM system, name and date of birth must be provided at checkout. 

3. Participation

The Program commences on 1st June 2025 (“Start Date”) and will be available whilst stocks last. NZMS will announce any closing date on its website but reserves the right to terminate the Program at any time. 

To receive one (1) free Dexcom ONE+ OR one (1) free Dexcom G7 Sensor, you must complete the sample request form, ensure the details provided including name, shipping address, and mobile phone number, are correct before submission. Delivery details cannot be altered after submission. 

By participating in the program, you acknowledge and agree that your data may be shared with third-party service providers, for the purpose of enhancing and delivering our services. This may include, but is not limited to, the use of your data for communication services, such as sending SMS notifications, verifying user identities, and improving customer support interactions. 

Once the form is completed, NZMS will check your details against its existing databases in line with the Privacy Policy to confirm that you are a new customer to Dexcom ONE+ OR Dexcom G7. If you pass the eligibility check, NZMS will confirm your order via email and deliver one (1) Dexcom ONE+ OR Dexcom G7 Sensor. 

By completing the form, you agree to be bound by these T&Cs. Any claims that do not comply with these T&Cs may be deemed invalid and disqualified. 

You confirm that if you are eligible for one (1) free Dexcom ONE+ OR Dexcom G7 Sensor, it shall not be sold, billed, traded, charged, or subject to any claim for reimbursement. 

There will be no cash alternative for the free sample. 

NZMS and Dexcom are not liable for any disruption to this Program for reasons beyond its reasonable control. In the event of disruption, NZMS will use reasonable endeavours to resolve the issue and resume the Program under the existing terms and conditions. Additionally, NZMS and Dexcom accept no responsibility for sample orders that are lost, mislaid, damaged, or delayed in transit, regardless of cause. 

By participating in the Program, you agree to be bound by these T&C. Any claims not complying with these terms may be deemed invalid and disqualified. 

By participating in the Program whilst stocks last, you acknowledge that:

• The Program is only open to eligible individuals listed under paragraph 2.
• Each eligible individual is only entitled to submit one (1) claim.

Completing the sample request form for more than one (1) time per person is strictly prohibited. Any such actions will be considered fraudulent, and NZMS reserves the right to withhold the relevant sample, report these activities to law enforcement, and pursue legal action. 

You undertake not to resell or distribute the sample under any circumstances. Unauthorized resale constitutes a breach of these T&Cs, and NZMS reserves the right to claim the current recommended retail price of the Dexcom ONE+ or Dexcom G7 Sensor as a debt. 

You represent and warrant that the details provided during checkout are correct. 

NZMS reserves the right to report any suspected fraudulent activity, including unauthorized multiple form completions, counterfeit information, or identity theft, to law enforcement and pursue legal action as necessary. 

In the event of a breach of the T&Cs, Dexcom may impose penalties or seek damages. 

7. General

To the extent permitted by law, NZMS, its agents, or distributors are not liable for any loss, damage, personal injury, or death resulting from the use of the sample, except when caused by Dexcom’s negligence. 

If there is any reason to believe that these T&C have been breached, NZMS may exclude you from participating in the Program. NZMS may disqualify any request who it reasonably believes has interfered with or attempted to interfere with the fair running of this Program. 

NZMS reserves the right to void, cancel, suspend, or amend the Program and this T&Cs as necessary. 

8. Data Privacy

By requesting the sample, you agree that your personal data supplied during this Program may be shared on to third-party suppliers solely for the purpose of this Program. You can find more information on our privacy policy. 

By participating in the Program, you agree to be contacted by phone and/or email by our Customer Service team to follow up on your sample experience and receive educational emails about the Dexcom ONE+ or Dexcom G7 system and provide feedback on your sample experience. 

9. Governing Law and Jurisdiction

This Program is governed by the laws of New Zealand, and participants submit to the exclusive jurisdiction of the courts of New Zealand.
